Fees & Payment Options

Initial Intake Session: $175

Ongoing Therapy Sessions (55-60 minutes): $140

I offer several payment options, including:

  • Self-pay (private pay)

  • Insurance

  • Out-of-network reimbursement through superbills

  • Limited sliding scale appointments based on availability and financial need

If you are using insurance, I am in-network through Headway and can bill your insurance directly for covered services. Coverage details such as copays, deductibles, and session limits are determined by your insurance plan.

If you are using out-of-network benefits, I can provide a superbill through SimplePractice for you to submit to your insurance company for possible reimbursement. Reimbursement depends on your specific plan, so I recommend reaching out to your insurance provider to learn more about your benefits.

Sliding scale appointments are offered on a limited basis and are intended for those with financial need. Please reach out if you would like to inquire about availability.

LMSW Clinical Supervision

I offer clinical supervision for LMSWs working toward licensure in a supportive, collaborative, and clinically focused space. Supervision is tailored to your needs and may include case conceptualization, treatment planning, ethics, documentation, and building confidence as a developing clinician.

Sessions are held virtually for flexibility.

Supervision Rate is $50 per hour.

Insurance

I am in network with the following insurance plans:

Using insurance for therapy is a personal decision that may involve both benefits and limitations. While insurance can help lower the cost of sessions, insurance companies often require certain information in order to authorize and process payment for services. This may include:

  • Dates of sessions

  • A mental health diagnosis

  • Occasionally, additional clinical information related to treatment

Please be aware that information submitted to insurance companies becomes part of your insurance record, and the level of confidentiality is ultimately determined by your insurance provider’s policies.

I encourage you to consider your comfort level with these requirements when deciding whether to use insurance benefits for therapy services.

To support accessibility, I offer a limited number of reduced-fee sessions using a self-selection scale available to all clients. This allows you to choose a fee that reflects your current financial situation.

Self Selected Fees & Access

From time to time, clients may request documentation such as treatment summaries, disability or FMLA forms, accommodation letters, or releases of information. These requests are considered carefully to ensure they are clinically appropriate and consistent with ethical and professional guidelines.

In general, documentation requests are addressed after we have established a therapeutic relationship (typically around 60 days or approximately six sessions). This allows for an adequate understanding of your needs and ensures that any documentation provided is accurate and clinically supported.

Where appropriate, documentation may be completed during a scheduled session. If additional time outside of session is required, fees will apply based on the type and complexity of the request.

Please note that completion of documentation is not guaranteed. All requests are reviewed on a case-by-case basis and may be declined if they fall outside of my clinical judgment, scope of practice, or ethical guidelines. If a request cannot be fulfilled, I may recommend working with your primary care provider or psychiatrist, as they may be better positioned to complete certain forms.

Letters, Forms & Records Policy

Records Requests

Clients have the right to access their clinical records. Requests must be submitted in writing to julia@moxietherapyct.com and will be fulfilled within 30 days in accordance with applicable regulations. There is no fee for releasing records directly to clients or authorized third parties.

While clients are entitled to their records, it is important to understand that the release of clinical documentation can carry both benefits and risks, which can be discussed in session if needed.

Fees for Documentation (when completed outside of session time)

  • Disability, FMLA, or structured forms/questionnaires: $150

  • Treatment summaries: $40 per page

  • Accommodation or ESA letters: $40 per page
